1. Program description
Alli Health sends text messages on behalf of participating healthcare practices as part of their Advanced Primary Care Management (APCM) programs. Messages are sent only to patients who have enrolled in their practice's APCM program and consented to be contacted. Message types include: monthly care check-ins, replies within an ongoing check-in conversation, scheduling and re-contact attempts, and program/service notifications from the patient's care team.
2. Opt-in
Consent to receive text messages is collected during APCM enrollment (by phone, in writing, or electronically) and documented in the patient's record with the practice. Enrolling in text check-ins is optional; patients may choose phone or email check-ins instead. Consent to receive texts is not a condition of receiving care from your practice.
3. Opt-out and help
- Reply STOP at any time to stop receiving text messages. One final confirmation message will be sent, after which no further texts will be delivered. Opting out of texts does not un-enroll you from your practice's APCM program. Check-ins can continue by phone or email.

4. Message frequency and rates
Message frequency varies; a typical enrolled patient receives approximately one to three messages per month, with additional messages if a conversation is in progress. Message and data rates may apply according to your mobile plan. Carriers are not liable for delayed or undelivered messages.
5. Do not use for emergencies
Text check-ins are not monitored continuously and must not be used for urgent or emergency needs. If you are experiencing a medical emergency, call 911. If you need to reach your care team urgently, call your practice directly.
6. Privacy
Text conversations are part of your care program and are handled as protected health information under HIPAA, pursuant to the Business Associate Agreement between Alli and your practice. See our HIPAA & BAA page. Mobile opt-in data and consent are not shared with third parties for marketing purposes. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
7. Supported carriers
Messages are supported on major U.S. carriers; delivery is subject to carrier availability.
