Help us be the ally.

Alli runs Medicare APCM for independent primary care: warm monthly check-ins with patients, supervised by their own care teams, funded by Medicare. We're early, moving fast, and hiring the people who'll define how this company sells, looks, and cares. Every application below is read by a founder.

Regional Sales Representative, Primary Care (APCM)

Sales & GTMRemote · territory-basedFull-time

The role

You'll own a territory of independent primary care practices and bring them onto Alli. You already know these buyers (the physician-owner, the practice administrator, the office manager) and you know how to earn their trust. This is a consultative, education-first sale: you're helping practices understand a reimbursement pathway most of them are leaving on the table, then showing them how Alli captures it without adding to their plate.

We're specifically looking for reps who already sell into this world, value-based care enablement, ACO/MSSP participation, care management (CCM/RPM/APCM), or clinical software for independent PCPs, and who bring existing relationships with practices that carry sizable Medicare panels.

What you'll do

  • Own and grow a territory of independent primary care practices with meaningful Medicare patient panels
  • Prospect, qualify, and build relationships with physician-owners, practice administrators, and office staff
  • Run discovery conversations and practice walkthroughs that connect APCM to each practice's specific patient population and economics
  • Manage the full sales cycle from first intro through signed agreement and go-live handoff
  • Consistently hit pipeline, conversion, and territory revenue targets
  • Feed field insights back to product and leadership; we're early, and your voice shapes the playbook

What we're looking for

  • 5+ years of healthcare sales experience, with a track record selling into independent primary care practices
  • Direct experience with value-based care, ACOs, care management (CCM/RPM/APCM), or practice-facing clinical/RCM software strongly preferred
  • Existing relationships with independent PCPs and their office staff: you can get a physician-owner on the phone
  • Comfortable selling to clinical and administrative buyers in the same deal
  • Fluent enough in Medicare economics to have a credible reimbursement conversation
  • Self-directed operator who runs a territory like it's their own business; startup-ready

Compensation

Competitive base plus uncapped commission; equity for the right early hires. Remote-first with travel within your territory. Specific numbers shared in our first conversation.

Founding Creative Designer, Brand & Product

DesignRemoteFull-time or contract-to-hire

The role

You'll be the first design hire and own how Alli looks and feels everywhere: the marketing site, the practice-facing product, sales collateral, and the brand system itself. We have a young brand with strong bones (a design system, a distinctive homepage, a clear voice) and we need the person who takes it from "impressive for a startup" to simply impressive. You'll work directly with the founders and ship constantly.

What you'll do

  • Own and evolve the Alli brand system (typography, color, componentry, motion) across web, product, and print
  • Design the practice-facing product experience: dashboards, escalation views, and the check-in review workflow
  • Produce the sales collateral: decks, one-pagers, leave-behinds that survive a front desk
  • Art-direct how an AI phone call is shown on screen and how an escalation is presented to a physician
  • Build fast, test with real practices, and refine; we ship weekly, not quarterly

What we're looking for

  • A portfolio with shipped brand + product work, ideally at an early-stage company where you were the design org
  • Strong systems thinking: you build reusable kits, not one-off screens
  • Comfort designing for skeptical, time-poor audiences (physicians and office managers, not tech early-adopters)
  • Working fluency in HTML/CSS or Figma-to-code tools, enough to ship without a handoff bottleneck
  • Healthcare experience is a plus. We're mainly hiring for taste and speed.

Compensation

Competitive salary and meaningful early-stage equity, or a contract-to-hire arrangement if you prefer to date before marrying. Remote-first.

Registered Nurse, Escalation Review

ClinicalRemotePart-time / PRN to start

The role

A human clinician reviews each flag Alli raises before it reaches a practice. You'd be one of those clinicians. You'll review flagged check-in conversations against patient history and trend, separate real urgency from noise, and route escalations to practices with clear, useful context. This role starts part-time/PRN as our pilot practices come online and grows with the panel.

What you'll do

  • Review flagged check-in transcripts and summaries against the patient's care plan, history, and trend
  • Triage: distinguish same-day escalations from routine care-team notifications
  • Write the escalation note: what the patient said, why it's a concern, and what's attached
  • Close the loop: confirm outcomes are documented and the next check-in reflects them
  • Shape our clinical review playbook, thresholds, and safety guardrails from the ground up

What we're looking for

  • Active, unencumbered RN license (compact/multi-state license a plus)
  • 3+ years in primary care, care management, telephonic triage, or chronic disease management
  • Excellent clinical judgment in ambiguous, text-based situations, and the discipline to escalate rather than advise
  • Clear, concise clinical writing
  • Comfortable working with AI-generated documentation and giving feedback that improves it

Compensation

Competitive hourly rate; flexible scheduling; a path to a full-time lead role as our practice panel grows.

Clinical Operations Associate

ClinicalRemoteFull-time

The role

You'll run the operational spine of Alli's care program: patient enrollment and consent, records moving cleanly between our platform and practice EMRs, monthly documentation completeness, and the billing-ready file every practice gets at month end. This is the role for someone from a practice back office (an MA, LPN, care coordinator, or practice-operations lead) who wants to set these processes up from the start.

What you'll do

  • Run patient enrollment and consent workflows with practices, and document them compliantly
  • Keep patient records synchronized between Alli and each practice's EMR
  • Monitor monthly check-in completion and chase down the gaps before they become billing problems
  • Prepare month-end billing-support files and walk practice billers through them
  • Turn what you learn into SOPs; you'll write the manual the next ten hires follow

What we're looking for

  • 3+ years in a primary care practice, care-management company, or clinical back office (MA, LPN, coordinator, or ops)
  • Hands-on EMR experience (eClinicalWorks, athenahealth, NextGen, or similar)
  • Familiarity with Medicare billing workflows; CCM/APCM exposure a strong plus
  • Checklist-driven and calm about deadlines
  • Comfortable being early. Plans change, and you write things down as you go.

Compensation

Competitive salary and early-stage equity. Remote-first.

Medical Director (Fractional)

ClinicalRemoteFractional · 8–10 hrs/week

The role

You'll be the physician voice inside Alli, the person who signs off on what our check-ins ask, where our escalation thresholds sit, and what "safe" means in practice. This is a fractional role for a practicing or recently practicing physician who wants to shape how AI supports primary care without leaving medicine. You'll set clinical policy, review the hard cases with our nursing team, and be the peer on the line when a practice's physician wants to talk to ours.

What you'll do

  • Own Alli's clinical protocols: check-in question sets by condition, escalation thresholds, and review standards
  • Provide physician-level review for edge cases our RN escalation team flags up
  • Take the physician-to-physician calls with practices: onboarding and protocol questions
  • Audit a sample of check-ins and escalations monthly; feed findings into product and prompt design
  • Advise founders on clinical strategy, safety posture, and what to build next

What we're looking for

  • MD/DO with an active license and board certification in internal medicine, family medicine, or geriatrics
  • 5+ years of primary care practice; experience with Medicare populations and chronic disease management
  • Prior medical-director, ACO, or care-management leadership experience preferred
  • Pragmatic about technology, uncompromising about safety: comfortable saying "no" to founders
  • Clear writer; protocols you author will be read by nurses, engineers, and regulators

Compensation

Competitive fractional retainer plus meaningful advisor-level equity. Flexible hours; grows with the company if you want it to.

Founding Data Scientist, Conversations & Outcomes

DataRemoteFull-time

The role

Alli's product generates an unusual dataset: thousands of structured monthly conversations with chronically ill Medicare patients, each one tied to flags, escalations, and outcomes. You'll be the first person whose whole job is that data. You'll measure whether our check-ins catch the things a physician would want to know about, set the alert threshold so practices aren't flooded with false alarms and we don't miss a real one, and over time build the evidence we'll show practices and payers.

What you'll do

  • Define and instrument the metrics we run on: reach rates, completion, flag precision/recall, escalation outcomes, time-to-intervention
  • Evaluate conversation quality at scale; build the eval harness for what a "good check-in" means, and run it against every model and prompt change
  • Tune escalation thresholds with our clinical team: fewer false alarms without missing the ankle-swelling moment
  • Build practice-facing reporting a physician-owner can read in ninety seconds
  • Design the observational studies that turn our operating data into publishable, citable outcomes evidence
  • Build all of this inside our HIPAA boundary. De-identify where you can, limit access to what each person needs, and keep an audit trail.

What we're looking for

  • 4+ years in data science with real healthcare data (claims, EHR, care management, or clinical operations)
  • Strong applied statistics: you know why difference-in-differences beats a before/after chart
  • Experience evaluating LLM or conversational systems strongly preferred (eval design, labeling pipelines, drift monitoring)
  • Fluent Python/SQL; comfortable owning the whole stack from raw events to the chart in the board deck
  • Careful with numbers. If we publish a figure, you should be able to show where it came from.

Compensation

Competitive salary and meaningful early-stage equity. Remote-first.
